Shlokas and meaning from Shri Bhagvat Maha Purana(Shri Bhagavattam)
    -------------------------------------------------------------------------------
    Shri Bhagvattam 10.38

måtyur janmavatäà véra dehena saha jäyate
adya väbda-çatänte vä måtyur vai präëinäà dhruvaù

O great hero, one who takes birth is sure to die, for death is born with the body. One may die today or after hundreds of years, but death is sure for every living entity.

    Shri Bhagvattam 10.39

dehe païcatvam äpanne dehé karmänugo 'vaçaù
    dehäntaram anupräpya präktanaà tyajate vapuù

When the present body turns to dust and is again reduced to fiveelements—earth, water, fire, air and ether—the proprietor of the body, the living being, automatically receives another body of material elements according to his fruitive activities. When the next body is obtained, he gives up the present body.

A soul is said to be non-destructible and it enters another material body when death happens in it's former one.
The rebirth of a human being is based on Karma and migration theory of Hinduism.
According to this simplified theory once a person is dead, the soul is required to be ignited or purified five times before converting into a child in next birth. Though this process is not visible to human eye, it is taken care by nature based on Karma theory.
