I don't have any idea regarding Lokas. But yes it's said that after death proper ceremony of death is done so that the soul of individual is not stuck between heaven and Hell. It's so what called 'Moksha' liberaty in Hinduism. As per my Knowledge.

Different sects and sampradayas teach this differently. So in that sense, there are no facts, just opinions. I can only give you my sampradaya's teachings as that's the only one I know at all, and even then, I'm most likely only holding a partial understanding.

Thee human you know as you will no longer exist. It dies, along with the physical body. It is the soul (atman) that continues. There is a certain period between births, and that's where the lokas are, not physical, but astral worlds. Depending on how you (the soul, not you, Ignorant) behaved this life, your seed karma and more, you will be with others of your relative ilk, and then when the time is right, you'll find a new body, and reincarnate.
